Pneumomediastinum after birth, without birth injury or resuscitation maneuvers, is an unusual situation that can present with grunting, deafening of cardiac sounds, and bulging of the hemithorax. If clinical condition allows, conservative approach is advised even if the pneumomediastinum does not spontaneously resolve in 1 or 2 days.

Spontaneous pneumothorax is a recognised cause of respiratory distress in the neonatal period. Spontaneous pneumothorax occurring during the neonatal period in a term neonate is rare. We report a case of symptomatic spontaneous pneumothorax associated with congenital heart disease which needed intercostal drainage and mechanical ventilation in our NICU unit.

We present a case of 32 year Caucasian male with gradual swelling of his face and neck with increasing hoarseness of voice. He was treated for anaphylaxis with little improvement. He underwent a video-assisted thoracoscopic surgery procedure (VATS) with a definite diagnosis of sub pleural bleb.
